
Through the efforts of the Alfred Saliba Family Services Center, staff members recognized that many of their clients and consumers lacked access to basic, fundamental medical services. A generous donation from the family of Dr. Paul Flowers soon made it possible for the Family Services Center to establish a medical clinic. It's primary mission was to augment the services offered by the Family Services Center, in an effort towards helping clients make positive lifestyle changes. The Family Health Clinic (FHC) was created in August of 2000 and has been helping patients ever since by offering services that, in some cases, would not have been otherwise available.
Who is eligible for medical care through Family Health Clinic?
Free medical care will be provided to families and individuals without medical insurance, who meet the Family Health Clinic's eligibility screening requirements.
FHC will
not see patients related to any of the following conditions:
Pregnancy
HIV
Birth Control
Auto Accidents
Workman's Comp
Disability Determination
(It is the intent of FHC to complement, not duplicate, available medical services)
Free medical care will be provided to any adult residing in Houston County who is without health insurance, Medicare, Medicaid, or VA benefits and with a total household income up to 150 % of federal poverty level. The Health Clinic does not provide physical examinations for employment or disability determination. Consumers will be seen by appointment only after first being screened for eligibility. Free medical care may be provided to individuals who reside outside of Houston County; but only if they are actively involved in other programs provided at the Alfred Saliba Family Services Center.
What must I provide to be eligible for free services?
For eligibility screening purposes, patients are asked to provide proof of the following:
Household income (Social Security, Child Support, Disability, Food Stamps, Unemployment, and Pay Stubs)
Picture ID with Social Security Number
Current Tax Return and/or Pay Stubs
Recent Utility Bills
Who will treat me?
FHC is staffed by physicians, a nurse practitioner, registered nurses, medical assistant, and support staff. The Clinic is also assisted by a dynamic group of volunteers selected from all areas of the community.
When is FHC open to the public?
Appointments may be scheduled any time during business hours by calling 673-3940 (Ext. 100)
Medication pick-up is scheduled for Wednesdays and Thursdays.

Tuesday/ Friday 8am to 12 Noon

The Family Health Clinic is a non-profit organization that seeks funding through grants, donations, and in-kind service. We are a United Way Organization.
